Appointment Checklist

  • If required, get a referral from your primary care doctor before your appointment and bring it with you.
  • Most major insurance plans are accepted. If required, get a pre-certification before your appointment.
  • Bring your insurance cards with you.
  • If you have had an imaging study at a location other than a Temple Health facility, bring a copy on disk.
  • When you arrive for your appointment, sign in at the reception desk.
  • If you need to change or cancel an appointment, please call at least 24 hours before your scheduled appointment.

You can schedule an appointment in two ways:

  • You can call us directly at 215-517-5000 between the hours of 8:00 a.m. and 6:00 p.m., Mondays – Fridays.
  • If you’d prefer us to contact you, please fill out the form below. This form is secure and our Access Center specialist will call you back within one business day to schedule your appointment.

Note: This appointment form should not be used for urgent medical issues or emergencies. Please call 911, your physician or go to the closest emergency room for care and treatment.
